Describe observed conditions
Identify the property and unit, the affected area and the conditions you have observed. Explain when the issue began, any action already requested from the owner or manager and whether the condition is ongoing.
Evidence and access
Provide dated photographs, relevant correspondence or notices. Keep originals. Tell the reviewer who can arrange access and whether there are concerns about safe contact. Do not enter another dwelling or attempt an unsafe repair to obtain evidence.
Review and limits
The City may seek information, arrange a review or refer the matter. This process does not decide a private damages dispute, cancel a lease or replace a court proceeding. Call 911 if there is an immediate danger, fire or medical emergency.
